Safety Recall Notice: Imperial EANx32 Recreational Dive Planner (RDP)

July 17, 2008

PADI Americas recently discovered an error in some printings of the English language, Imperial EANx32 RDP. No other version of the RDP is affected. Although the risk of injury is exceedingly remote and no injuries have been reported, this table is being recalled.
The affected Imperial EANx32 RDP has the following product number and product code (found in the lower right corner on the back of the table).

The product code digits indicated by “X” may vary. The last four digits “PDK7” indicate a recalled table, irrespective of what the first three digits are.
As a double check, on Table 3, check the 110 foot row under Pressure Group G. The table should indicate a Residual Nitrogen Time (upper number in white) of 13 and an Actual Bottom Time (lower number in green) of 12. If this box is blank, you have a misprinted table.

If you have an EANx RDP (product number 60049) with the production code of XXXPDK7 and/or a blank box where described, please return it to your PADI Dive Center/Resort or return it to PADI Amercia at the address below, as soon as possible. Limited Recall of Apeks TX, ATX and XTX 2nd Stages

This is limited to those 2nd stages/octopus that have not undergone an authorized annual service.

It has come to our attention that an incorrectly assembled 2nd stage has been purchased. If you have an Apeks regulator or octopus that has never had an authorized annual service, regardless of how recently purchased, please take the regulator/octopus to your Apeks dealer to be visually inspected.

All Apeks dealers and distributors have been made aware of this issue. Do not dive with the regulator until it has been visually inspected. Apeks apologizes for any inconvenience this may cause. We are dedicated to making the world’s best diving products and ensuring diver safety at all costs.

Aqua Lung Announces Regulator Recall

RECALL INVOLVES CERTAIN TITAN/CONSHELF DIN HANDWHEEL RETAINERS

Aqua Lung was recently informed about the breakage, under pressure, of a TITAN/CONSHELF DIN HANDWHEEL RETAINER weakened during service by manual torque loads superior to the recommended values.

Aqua Lung asks all the owners of TITAN DIN regulators whose serial number is lower than 6062501 or TITAN/CONSHELF DIN adaptors that are marked 300 BAR MAX to return their regulator to their Aqua Lung authorized retailer for an upgrade.

The corrective action is replacement of the brass DIN HANDWHEEL RETAINER by another stainless steel DIN HANDWHEEL RETAINER marked 300 BAR MAXI.

This replacement is made under warranty.

All the TITAN DIN regulators whose serial number are above 6062501 and\or whose DIN handwheel retainer is marked 300 BAR MAXI are assembled with a stainless steel DIN handwheel retainer and are not affected by this recall.

Poseidon Issues Product Safety Notice - Recall

July 2008

Potential defect discovered in certain Poseidon BCD Models
A potentially serious defect has been detected in a certain production run of the Poseidon W50 Bladder used in conjunction with both the platform and BeSea model BCDs. This flaw deals with a weld in the internal air cell and it is possible that this weld could fail during a dive. Should this occur, the BCD will loose its ability to hold air and provide flotation and buoyancy control during a dive without warning, putting the diver at significant risk of serious injury or death.

Both Poseidon Diving Systems and their US distributor, Waterproof Gear, Inc., are taking rapid action to correct this problem without any cost to the consumer. This defect only involves bladders identified by the lot #5445. The lot number is located on a tag permanently attached to the internal air cell beneath the zipper closure of the wing. The lot number is a 4 digit number located at the bottom of the tag to the left. Please see the graphic, and note the lot number underlined in red. These bladders did not enter the market until September 17, 2007, and any bladders sold prior to that date are not impacted and do not need to be checked. All bladders that meet the above description should be considered a serious safety risk to the diver, should not be used under any circumstances, and should be immediately returned for replacement.

Although not every bladder in this lot is adversely effected by this defect, there is no way to tell which bladders are impacted. Therefore, every bladder identified from lot #5445 should be returned to an authorized dealer for replacement.

Consumers who may have this product are directed to return the product to their local authorized Poseidon Dealer immediately. Do not dive with this product and do not attempt to analyze or repair any potential defect in the product yourself. The product must be returned for identification by an authorized technician who will return any affected units to the Poseidon Distributor for repair. . If you have one of these W50 wings and do not have a local authorized Poseidon Dealer, contact our customer service department for directions on returning your product for repair at 727-944-5675. All repairs will be completed without cost to the consumer.

Dealers should return the entire W50 wing assembly to Waterproof Gear, Inc. for replacement of the internal air cell. There is no need to return the harness assembly or any other parts of the BCD. The wings should be clearly tagged with the dealer’s identifying data and the customer’s name as the external portion of the wing will be returned to your customer. The dealer should check to verify that the wing meets the description above before returning it for replacement. Once you have identified that you have an affected product, box the product for shipment, and contact our customer service department to arrange for pickup of the product.

Name of product: Swivel for a Scuba Regulator

Units: About 170

Importer: Innovative Scuba Concepts Inc., of Colorado Springs, Colo.

Hazard: The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete and sudden loss of the diver's air supply, causing the diver to engage in emergency ascent procedures. This poses a risk of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent procedure fails.

Incidents/Injuries: The firm has received one report where the swivel separated during a dive and caused the diver to ascend using the buddy breathing technique. No injury was reported.

Description: The recall involves the HO110 Swivels sold as an aftermarket regulator component. There are no markings such as a date or production code embossed on the unit. Contact Innovative Scuba Concepts for information on where the recalled units were sold.

Sold at: Specialty retail dive stores nationwide from January 2006 through March 2007 for about $40. They also could have been installed by a dive shop regulator technician.

Manufactured in: Taiwan

Remedy: Consumers should stop using regulators with the swivel attached and contact the dive store where purchased for a refund.

Scubapro® Recalls MK 20 Scuba Regulators for Service

The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.
Name of Product: Scubapro® MK 20 First Stage Regulators

Units: About 48,500 units

Manufacturer: Scubapro® USA, of El Cajon, Calif.

Hazard: Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard. Incidents/injuries: No injuries or underwater incidents have been reported. Scubapro® USA has received eight reports of units that failed after being serviced.
Description: The recall involves first stage regulators that are marked with the Scubapro brand and identified as the MK20.

Sold at: Authorized Scubapro® dealers nationwide from January 1996 through December 2001 for about $260.00USD.

Manufactured in: Italy and United States

Remedy: Scubapro® USA has been providing a service upgrade kit via the authorized dealer network since May 2005 which contains a Yoke or Din retainer that prohibits over-tightening during annual service. Consumers can easily identify if the service upgrade has been completed by counting the vents on the plastic saddle located on the regulator. If the service upgrade has not been completed, the consumer should bring the regulator to any authorized Scubapro® dealer to have the service upgrade completed free of charge.

Lawrence Factor Recall

September 2006
There has recently been a recall on Lawrence Factor X14249 filters.
If you have any of these Lawrence Factor filters please call Lawrence Factor (305)430-0550 immediately for a replacement filter.

If you own an Alkin W31 (Primary Unit Using LF x14249) and are using the Lawrence Factor disposable filters for breathing air, you have a problem!
The problem is not as big if some OTHER filter is in use after the LF x14249 (e.g. a Hyperfilter), but it is a potentially huge problem if one is filling cylinders directly off the compressor.

Here's Why - LF was apparently given the wrong flow direction for the gas in the can. That is to say, that the gas encounters the Hopcalite (CO converter) first instead of last, then the activated carbon (the contaminent filter) and finally the dessicant (13x). This is exactly backwards!

This is serious because neither Hopcalite or activated carbon can/will do anything once they get saturated with water vapor. Without the 13x in front of them, they will fail in very short order, as in single-digit hours of operation after being installed. The Hopcalite will be rendered useless immediately.


IMPORTANT SAFETY NOTICE REGARDING Suunto D9 AND D6 DIVING INSTRUMENTS

June 9, 2006 – Suunto Oy today announced an important safety notice regarding its D9 and D6 diving instruments. This global decision involves D9 and D6 instruments that have been delivered to distributors and retailers, as well as product that has been sold to consumers.

Quality and safety are paramount concerns for Suunto Oy. Our testing program for the D9 and D6 product by Suunto employees and independent divers all over the world has been extensive. Despite this, Suunto Oy has identified a software bug in the D9 and D6 instruments. The software bug may cause the D9 and D6 to incorrectly track dive time on rare occasions, potentially causing a risk to the diver.

The products affected are:
D9 serial numbers 62102582 and below
D6 serial numbers 62103693 and below


The software bug may activate when the D9 and D6 automatically recalibrate to keep the correct time, or the user manually updates the seconds in set mode. Suunto approximates that there is 0.6 % (a six-tenths of one percent) chance that the software bug may appear during a dive. When the dive time is incorrect, every fourth second is not registered in the dive time and surface interval time, i.e., dive time goes 25% slower than actual time and tissue loadings are recorded at 25% less than actual loadings. As a trained diver you understand that this could affect your decompression requirements and that the length of a dive is one of the key factors in determining those requirements.

Even though the D9 and D6 will function correctly approximately 99.4% of the time during a dive, Suunto wants to eliminate the very small possibility of recording the dive time incorrectly. Therefore, Suunto is requesting that all D9 and D6 shall be returned for a software update that eliminates this issue. To date, there have been no known injuries related to this issue.

Action that YOU should take

• Suunto strongly recommends that you should not use the D9 or D6 for diving without backup instrumentation before the unit has been updated with the latest software. Please bring your D9 and/or D6 to your nearest local authorized Suunto retailer for the software updated as soon as possible.
• Please contact your local authorized Suunto retailer – for contact information please visit our web site www.suunto.com You will then be advised on how to bring your D9 and/or D6 to the retailer for a software update. Once the retailer receives your D9 and /or D6 they will then forward your unit to the Suunto Service Center for the software update. You will be advised by the retailer the approximate turn around time that the process will take.
• If that is not possible or if you require any further details, contact the Suunto Help Desk at SuuntoD9-D6@nordictelecenter.fi To contact Suunto by phone, call toll free 1 800 543 9124 in the USA and Canada, or +358 284 1160 (international call rates are applicable) from all other countries.
• On receiving your D9 and /or D6 back, you will notice that there will be a small white dot on the rear. This identifies that the unit has been updated. As a complimentary service Suunto will at the same time replace your battery and perform a pressure test free of charge.

Scubapro X650 Diving Regulators Recalled for Drowning Hazard

Name of Product: Scubapro X650 Second Stage Regulator

Units: About 670

Manufacturer: Scubapro with global headquarters located in El Cajon, Calif.

Hazard: A manufacturing error could cause the main housing of some regulators to change shape over time, which could cause the cover and diaphragm to become dislodged. If this occurs, air flow will be interrupted and the regulators will no longer function, posing a drowning hazard to users.

Incidents/Injuries: None reported.

Description and Models: The recall involves some X650 second stage regulators. All X650 second stage regulators are clearly marked with a blue capital “X” next to the numbers “650” in white. Affected regulators have a visible bump located on the right side of the unit between the letter “C” and where the mouthpiece attaches to the regulator body.

Sold at: Authorized Scubapro dealers in the U.S. from June 2005 through May 2006 for between $400 and $670. Manufactured in: Genova, Italy

Remedy: Consumers should stop using the product immediately and contact Scubapro to determine if they have an affected second stage regulator. If so, they will receive a free replacement regulator. The company is contacting registered owners of the product directly.

Ocean Management Systems Scuba Manifold Recall

Issued November 2004

The following product safety recall was conducted voluntarily by the firm in cooperation with the Consumer Product Safety Commission (CPSC). Consumers should stop using the product immediately unless otherwise instructed.

Name of Product: Rotating Cross Bar Scuba Manifold

Units: About 140

Importer: Ocean Management Systems, Inc., of Middletown, N.Y.

Hazard: Undersized O-rings installed on the cross bar can cause a loss of breathing air from the scuba cylinder, presenting a risk of drowning.

Incidents / Injuries: There has been one report of breathing air leaking from a cylinder. No injuries have been reported.

Description: The recall involves the 200 and 300 Bar versions of the OMS Rotating Cross Bar Manifold. The manifold consists of two valves which have "O.M.S." stamped on the side of each valve and a crossbar that has three O-rings per side. Each part has a black plastic knob.

Manufactured in: Italy

Remedy: Affected units will be replaced at no cost to the consumer.

Manufacturer: Halcyon Manufacturing, of High Springs, Fla.

Hazard: The SCUBA BC bladder can have a slow leak because of imperfections within the machining of the stainless air barrel of their inflators. This can cause unexpected buoyancy problems with divers, possibly resulting in decompression sickness.

Incidents/Injuries: Halcyon has reports of 10 incidents of BCs leaking. No injuries or deaths have been reported.

Description: Halcyon is recalling all stainless steel power inflators. They have a stainless steel oral and power inflate button. Inflators made of plastic are not included in this recall. The repaired inflators have a groove at the base of their retainer nut. The BCs sold with these inflators are black and the Halcyon logo is on the front and collar of the BCs.

Sold at: Dive stores nationwide sold these inflators individually from October 2003 through May 2004 for about $46. The inflators also were sold as part of BC units ranging from $270 to $600.

Remedy: Consumers should stop using these BCs immediately and contact Halcyon for information on receiving a free repair.

Oceanic Issues Important Consumer Product Bulletin

BC Inflator mechanisms exhibit problems that have been corrected by re-designed parts. Company urges consumers to be aware and bring affected products to their local Oceanic retailer for repair.

OCEANIC CONSUMER NOTICE STOP PRODUCT USE
BUOYANCY COMPENSATOR INFLATORS


Oceanic has been informed by its supplier that they have received isolated reports of BC Inflator mechanisms exhibiting continual flow of air into the BC after release of the power inflation button.

Affected are Oceanic Reliant model Inflators that are fitted on various models of Oceanic's BCs that were sold between February 1, 2004, and June 24, 2004. BCs sold prior to February 1, 2004, do not have these Inflators and are not affected.

Testing and evaluation has shown that the Power Inflator Button may stick in the depressed position. Engineers have designed new replacement parts that will prevent the Button from sticking.

Oceanic asks that all owners of Oceanic BCs that have Reliant Inflators return them to an Authorized Oceanic Dealer for a retrofit at no charge.

WARNING: DO NOT CONTINUE USE of the BC or attempt to disassemble any components of the Inflator. Take the BC with Inflator to your nearest Authorized Oceanic Dealer for inspection and service. Only Authorized Oceanic Dealers and their trained service technicians must perform this retrofit. Non-authorized personnel who attempt the retrofit procedure risk voiding the products warranty and rendering the product dangerously unsafe for normal SCUBA use.

RECALL: Light & Motion Sunray Mod Light
DATE: July 30, 2003

Effective immediately, Light & Motion announces a Safety Recall on the Sunray Mod Light.

There have been a few incidences in which the light inadvertently turned on while unattended.

It has been determined that if the light accidentally becomes partially flooded, it is possible for the light to turn on by itself.

If you suspect the light has been flooded, it is important to unscrew the silver bezel and remove the light bulb from the socket. Light & Motion is taking the cautious route and issuing a Safety Recall and is requesting that the lights be sent in for upgrading.

ZEAGLE RECALL: Flathead VI, 50D, Envoy, DS IV Regulators

Zeagle Systems, Inc. is recalling 931 first stage Scuba regulators sold between January 20, 2003 andMay 19, 2003. If you have one of these regulators it is essential to take it to your dealer for immediate repair.

Do not attempt to dive with this regulator again until the regulator has been inspected by an authorized Zeagle dealer.

These first stage regulators may have a defective high-pressure seat that could cause the first stage to over pressurize. If this occurs, the air supply to the diver could be interrupted, making it impossible for the diver to breathe from the regulator. This type of failure is most likely to occur upon initial pressurization of the regulator and is unlikely to occur underwater.
